DESCRIPTION:

 

To provide initial and on-going education and training to Health Information Management Professionals at KHSC for clinical data collection and abstracting.  To monitor and assess the impact of changes in data classification standards on departmental practices.  To address internal coding queries and source out expert guidelines to respond to the queries. To identify data quality issues, conduct audits and institute education as required for both sites.  To develop and conduct education sessions related to data collection and quality. To ensure the completeness, quality and accessibility of accurate health record information by coding, analyzing, and abstracting data from the patients’ records. To conduct audits and provide interpretation of the data in the Med2020 Winrecs and Oncology abstracting computer system. To ensure NACRS Level 1 and 3, DAD, OMHRS, Trauma, CCO deadlines are met. To code and abstract data from the patients records which will provide support for  statistical analysis, research and peer review by hospital staff,  Ministry of Health,  CIHI, and Cancer Care Ontario.

 

 

PRINCIPLE DUTIES & RESPONSIBILITIES:

• Coding of DAD, NACRS and Oncology cases: to accurately assign diagnosis and intervention codes according to the ICD-10-CA, CCI and AJCC staging classifications.  Follow established coding guidelines from CIHI, WHO, CCO and the Hospital. To accurately abstract defined data elements from the patients’ charts into the Med2020 computerized Winrecs/Oncology abstracting programs in accordance with CIHI, OH-CCO and the hospital’s requirements
• To provide a methodological approach to internal quality assurance audits and to perform these audits. To extract, analyze, and interpret data from the Winrecs, Quadramed PCS, OPIS and ARIA systems. To be proficient in report writing using Crystal reports. To implement measureable assessment tools, data quality documentation policies and procedures and education as required. To ensure workload verification for all Oncology programs including Systemic, Radiation, PSO and Palliative.
• To prepare, edit and correct the abstract data for accurate and timely submission to CIHI and CCO. To perform quality assurance audits to maintain data integrity. To ensure the timely electronic transfer of the abstract data to CIHI and ALR files to OH-CCO from HDH site and KGH site after completion of the month-end process.  To correct any errors and submit corrections electronically. Dissemination of updates on Clinic changes in Oncology Program and OH-CCO requirements.
• To assist the Manager and Director with development and updating the clinical coding and abstracting policies, procedures and standards
  • To disseminate and communicate all new or revised policies applicable to Health Information Services staff.
  • To update the Reference Manuals and literature for the HIM professional staff at both sites. To work towards standardized data definition and work processes.
• To develop and conduct as required, education sessions for:
  • Clinical staff awareness re the role of the HIS Department in the capture and dissemination of quality clinical information. 
  • Orientation and training of physicians on case mix groups, RIW’s, AJCC staging, complexity levels and chart completion.
  • Internal education opportunities that will enhance the coding and abstracting skills of the Health Information Management professionals.
  • HIM practicum students
• To organize and facilitate regular meetings with the HIM staff to address coding queries, problems and concerns.  To source out expert guidelines to respond to the queries.  To present the results of the quality assurance audits and ensure corrective action is taken. To regularly provide coding expertise to the Information Analysis and Distribution Department analysts to ensure appropriate abstract data element selection. To review the data generated by IAD in order to identify and assess any data quality issues.
• To test, validate and implement Med2020/Oncology/Treat/Collector/OPIS/Quadramed PCS/ARIA systems software updates with the support of IS as required.  To maintain Winrecs/Oncology/Treat/Collector system tables and files as required.

QUALIFICATIONS:

• Graduate of an approved Health Information Management program – Community College or Canadian Hospital Association Correspondence Program.
• Certified active member of the Canadian Health Information Management Association  (CHIMA)
• Three years coding experience for DAD, NACRS and Oncology as a Health Record Practitioner in an acute care institution.
• Demonstrated ability and knowledge with ICD-10-CA, CCI and AJCC staging, OH-CCO coding practices and abstracting guidelines as well as familiarity with CIHI reports
• Extensive knowledge of Case Mix Groups, Complexity and Intensity Weights
• Demonstrated ability with Med2020 Winrecs/Oncology/Treat/Collector computerized abstracting systems.
• Computer literacy and keyboarding skills with Windows, Microsoft Office Suite – Excel, Access, Word, Powerpoint.
• Demonstrated ability and knowledge in using Crystal Report writing and/or concepts of data analysis and database management.
• Thorough knowledge of the functions within the Health Information Services Department.
• Knowledge of the Public Hospitals Act, Mental Health Act, Personal Health Information Protection Act
• Excellent interpersonal, oral and written communication skills.
• Demonstrated ability to communicate effectively with physicians and hospital staff.
• Must safeguard the confidentiality of patients’ personal health information.
• The ability to show initiative and work in an independent, organized manner.
• Proven ability to attend work regularly.
• Satisfactory criminal reference check and vulnerable sector search required.

 

PHYSICAL REQUIREMENTS:

The incumbent will be required to meet the physical demands as described for the area of work.
